The Cholesterol Number Your Standard Panel Doesn't Check
Most of what we've covered in the Cura Check framework responds to lifestyle. Blood pressure moves with activity and salt intake. LDL moves with diet and statins. Lp(a) doesn't work that way. It's set almost entirely by your genes early in life and stays roughly flat for decades regardless of what you eat, how much you exercise, or how disciplined you are. That's precisely why it matters and why it's so easy to miss, the standard advice that works for LDL does nothing here.
Why it's not on your standard panel
A basic lipid panel checks total cholesterol, LDL, HDL, and triglycerides. Lp(a) requires a separate, specific test, and it isn't routinely ordered because current guidelines generally recommend checking it once in a lifetime rather than repeatedly, since it doesn't fluctuate the way other markers do. That "once and done" nature means most people never think to ask for it, and most routine physicals don't include it by default. Why it matters
An elevated Lp(a) level is an independent contributor to cardiovascular risk, separate from and in addition to LDL cholesterol. Someone can have a low LDL and a low PREVENT score and still carry meaningfully elevated cardiovascular risk if their Lp(a) is high, because standard risk calculators, including PREVENT, don't currently incorporate it as an input. This is one of the real edges of even a good, guideline-based risk score, it's built on the data most consistently available across large populations, and Lp(a) testing hasn't historically been common enough to be part of that.
What you can actually do about it
Here's the honest, evidence-tiered picture. Lifestyle changes don't meaningfully move Lp(a), that's the frustrating part. But knowing your number changes how aggressively the rest of your cardiovascular risk profile should be managed. If your Lp(a) is elevated, guidelines generally support being more aggressive with the levers that do work, LDL and ApoB lowering, blood pressure control, not smoking, since those remain fully effective regardless of Lp(a) status. There are also new drug classes specifically targeting Lp(a) currently in clinical trials, worth knowing about if your number comes back high, even though none are yet broadly available.
The other genetic factor worth knowing: familial hypercholesterolemia
A related, separate condition worth a quick mention. Familial hypercholesterolemia is an inherited condition causing very high LDL cholesterol from a young age, often well before any lifestyle factors would explain it. It's estimated to affect roughly 1 in 250 people, and it's substantially underdiagnosed, because very high LDL in someone young and otherwise healthy doesn't always trigger further questions. A simple flag worth checking yourself: if a parent or sibling had a heart attack or was diagnosed with high cholesterol at an unusually young age, it's worth mentioning to your doctor specifically, since family history is often the first real clue.
What to do with this
Ask your doctor whether an Lp(a) test makes sense as a one-time addition to your next panel, particularly if you have a family history of early heart disease that isn't fully explained by your other numbers. If it comes back elevated, that's not a reason for alarm, it's information that should sharpen how aggressively you and your doctor manage everything else that is modifiable.
