Cura Well Plan provides general health information and educational content. It is not a substitute for professional medical advice, diagnosis, or treatment. Always seek the advice of a qualified physician or other health provider with any questions about a medical condition. Never disregard professional medical advice, or delay seeking it, because of something you read or generated here.

Not a medical provider

Cura Well Plan is not a healthcare provider, does not practice medicine, and does not provide medical advice, diagnosis, or treatment recommendations. Content generated through Cura Well Plan, including suggested questions, summaries of research, risk scores, and resource recommendations, is informational only and should be discussed with your own care team before acting on it. No physician-patient, therapist-patient, or other professional relationship is created by using this site.

What these terms cover

These terms cover Ask Cura, Cura Check, bill explanations, guides, suggested doctor questions, summaries, emails, and printable or downloaded materials. None is a clinical interpretation, a second opinion, or an individualized care plan. Cura supports physical-health education, not mental-health assessment, counseling, or treatment.

No emergency use

Cura Well Plan is not equipped for medical emergencies. If you or someone you're caring for is experiencing a medical emergency, call 911 or go to the nearest emergency room. Do not use this site for emergencies.

Questions, uploads, and emails are not monitored for emergencies. We do not contact emergency services or your care team for you. A reassuring answer, risk estimate, or absence of a warning does not mean that a symptom or result is safe. Do not wait for a reply before seeking urgent care.

Who may use Cura Well Plan

You must be at least 18 years old to use Cura Well Plan. By using the site you confirm that you are 18 or older and that you agree to these terms. If you are using Cura Well Plan on behalf of someone you care for, you are responsible for having the authority to do so.

Cura Check risk scores and lab uploads

Cura Check produces an educational estimate, not a diagnosis, screening result, or clinical recommendation. Specifically:

• Cardiovascular estimates are produced using published population risk equations. A population estimate describes groups of people with similar inputs. It does not predict what will happen to you.
• The dementia tier and cancer factor checklist are educational summaries of common risk factors, not validated clinical scores, and are not a screening test.
• Results depend entirely on the values you enter or confirm. Values read from an uploaded document may be extracted incorrectly; you are responsible for confirming them against your actual report before relying on anything.
• Nothing in Cura Check should be used to start, stop, or change any medication, treatment, screening schedule, or exercise program without talking to a clinician. The daily action ladder is general wellness information, not a prescribed exercise plan.
• Cura Check output is not intended for use in insurance underwriting, employment decisions, or legal proceedings.

Uploaded files are read to extract lab values and then discarded. They are not stored. Only the values you confirm are ever saved, and only if you create the optional Cura Check account described in the Accounts section.

Bill help referrals

Through our Bill Help page, Cura Well Plan may introduce you to an independent professional who helps with insurance denials, billing errors, prior authorization disputes, surprise bills, or financial assistance applications. By submitting a Bill Help request you agree to the following:

• Referral only. Cura Well Plan is not a patient advocacy service, law firm, billing service, collection agency, or insurance broker, and does not provide legal, financial, billing, or insurance advice. We do not file appeals, negotiate bills, or act on your behalf.
• Independent professionals. Professionals we introduce are independent third parties, not employees or agents of Cura Well Plan. They are solely responsible for their services, qualifications, licensing where required, conduct, and results. We do not guarantee availability, response times, or that a match will be found for every request.
• Separate agreement. Any engagement is between you and the professional under their own terms. Review any proposed engagement before agreeing. An introduction does not create an agency or professional relationship with Cura Well Plan.
• No guarantee of outcome. We do not guarantee that any denial will be overturned, any bill reduced, or any savings recovered. You remain responsible for meeting insurer and provider deadlines, including appeal deadlines, and for paying amounts you legitimately owe.
• What you share. Share only what is needed to describe your issue. Do not send full insurance member IDs, Social Security numbers, or payment card details through the form.
• Limitation. To the fullest extent permitted by law, Cura Well Plan is not liable for any act, omission, advice, fee, or result of a third-party professional. The limitation of liability and indemnification sections below apply to Bill Help.

Accounts

Most of Cura Well Plan needs no account. Ask Cura, the guides, and the checklists work without signing in. The one exception is Cura Check: you can create an optional account to save your check results over time. A Cura Check account holds only your email address, the values you confirm, and the reports you generate. To delete your account and saved reports, email hello@curawellplan.com.

AI-generated content

Some content delivered through Cura Well Plan is generated using artificial intelligence. While built to be accurate and helpful, AI-generated content may contain errors, omissions, or outdated information. Cura Well Plan encourages every user to verify important information with their care team and not rely solely on AI-generated content for medical decisions.

Answers, citations, links, calculations, and document readings can be incomplete or incorrect, even when they sound confident. A cited source does not guarantee that an answer accurately describes the source or applies to you. Confirm original reports, units, reference ranges, deadlines, and important claims with the appropriate qualified professional. Do not start, stop, or change treatment based on Cura.

User-submitted content

You are responsible for the accuracy of any information you choose to submit, whether you type it, paste it in, upload a document, or snap a photo of a report. Do not submit documents or photos containing identifying information about other people without their consent.

You retain ownership of what you submit. You grant Cura Well Plan a limited license to process it solely to generate your results and operate the service, consistent with the Privacy Policy. We do not use your submissions to train models, and we do not sell them.

Acceptable use

You agree not to:

• Use Cura Well Plan to provide medical advice or clinical services to other people, or to substitute for clinical judgment in a professional setting
• Upload another person's medical records without their consent, or submit information you are not permitted to share
• Scrape, resell, republish, or build a competing product from the site's content or outputs without written permission
• Attempt to breach security, access other users' data, probe the service for vulnerabilities outside our responsible-disclosure process, or interfere with the site's operation
• Use the site for anything unlawful, harassing, or deceptive

Confidentiality and privacy

How Cura Well Plan logs anonymous questions, and how email addresses you choose to share are used, is described in the Privacy Policy, which is part of these terms.

Cura Well Plan operates as a standalone educational service, not as your healthcare provider or health plan, and does not represent that using the site provides HIPAA protections or medical confidentiality. Information you send us is subject to the Privacy Policy and applicable law. No legally privileged relationship is created. Do not send information you would not want to leave your device, and be aware that no internet transmission is completely secure.

Third-party links and resources

Cura Well Plan links to outside organizations, nonprofits, lab providers, and research sources for convenience. We do not control those sites, do not endorse every statement or service they offer, and are not responsible for their content, pricing, accuracy, or privacy practices. Your dealings with any third party are solely between you and that party.

Intellectual property

The site's text, design, structured guides, and software are owned by Cura Well Plan or its licensors and are protected by copyright and other laws. You may read, print, and share pages for your own personal or caregiving use. Any other reproduction, distribution, or commercial use requires written permission.

No warranty

To the extent permitted by applicable law, Cura Well Plan is provided "as is" and "as available" without warranties of any kind, express or implied, including but not limited to warranties of accuracy, completeness, currency, merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the site will be uninterrupted, error-free, or secure, or that any information, question set, or risk estimate is accurate or appropriate for your situation.

Assumption of risk

You use Cura Well Plan voluntarily and at your own risk. Decisions about your health, including whether to seek care, which tests to request, how to interpret results, and what to change in your daily routine, are yours to make with a qualified clinician. Any physical activity suggested in the daily action ladder carries inherent risk; consult a clinician before beginning or changing an exercise program.

Limitation of liability

To the fullest extent permitted by law, Cura Well Plan, its operator, founder, contributors, and service providers will not be liable for any indirect, incidental, special, consequential, exemplary, or punitive damages, or for any loss of profits, data, or goodwill, arising out of or related to your use of, or inability to use, the site, any content or risk estimate it produces, any error or omission in that content, or any reliance placed on it, whether based in contract, tort, negligence, strict liability, or any other theory, even if we have been advised of the possibility of such damages.

To the fullest extent permitted by law, our total aggregate liability together with the parties listed above, for all claims relating to the site and its materials will not exceed the greater of (a) the amount you paid Cura Well Plan in the twelve months before the claim arose, or (b) one hundred U.S. dollars ($100). Cura Well Plan is currently provided free of charge.

Some jurisdictions do not allow the exclusion of certain warranties or the limitation of liability for incidental or consequential damages, so some of the above may not apply to you. Nothing in these terms excludes or limits liability for fraud, fraudulent misrepresentation, gross negligence, willful misconduct, death or personal injury caused by negligence, or any other liability, to the extent applicable law prohibits that exclusion or limitation. Nothing waives mandatory consumer rights or remedies. These protections take priority over any conflicting warranty, risk, liability, or indemnity wording here.

Indemnification

To the extent permitted by applicable law, you agree to indemnify Cura Well Plan and its operator, founder, contributors, and service providers against third-party claims and reasonable, documented costs caused by your unlawful use of the site, intentional violation of these terms, or submission of information you had no right to share. This does not apply to ordinary personal or caregiving use, or to the extent a claim results from our own negligence or misconduct. It does not require you to waive your claims or mandatory consumer rights.

Governing law and disputes

These terms are governed by the laws of the State of New York, without regard to its conflict-of-laws rules. Any dispute that cannot be resolved informally will be brought exclusively in the state or federal courts located in New York, and you consent to that jurisdiction. This choice of law and courts applies only to the extent permitted by applicable law. It does not remove mandatory protections or a right to bring a claim in your local courts where the law gives you that right. You may email us to seek an informal resolution, but this is not required before seeking urgent relief, filing a claim, or exercising a legal right. These terms do not impose arbitration or waive class-action rights.

Severability and entire agreement

If any provision of these terms is found unenforceable, that provision will be limited or removed to the minimum extent necessary and the remaining provisions will stay in full effect. Our failure to enforce a provision is not a waiver of it. These terms, together with the Privacy Policy, are the entire agreement between you and Cura Well Plan regarding the site.

Suspension and termination

We may suspend or discontinue the site, or any feature of it, at any time, and may restrict access if these terms are violated. You may stop using the site at any time.

Changes to these terms

Cura Well Plan may update these terms periodically. Material changes will be reflected with an updated effective date at the top of this page. Continued use of the site after changes constitutes acceptance of the updated terms, to the extent applicable law recognizes that acceptance.

Changes apply prospectively, not to disputes that arose before the change. Where the law requires notice or affirmative consent, we must obtain it. Liability, intellectual-property, and dispute provisions survive termination only to the extent applicable law permits.
