Denial Appeal Packet Terms of Service
Effective date: October 23, 2026
What this service is
The Denial Appeal Packet is a document preparation and organization service offered through Cura Well Plan. We build an appeal packet from the documents you provide: an appeal letter, a deadline sheet, an indexed evidence appendix, and a submission checklist. We are not a law firm. We are not lawyers, medical providers, or insurance professionals, and nothing in the packet or in our communications is legal or medical advice.
This service is for denied claims of $3,500 or more.
No guaranteed outcome
Your insurer decides your appeal. We prepare the strongest packet we can from your documents, but we do not promise that an appeal will be approved, and no statement on this site should be read as a promise of a result.
Your responsibilities
The packet is only as accurate as the documents you send us. You are responsible for sending complete and accurate documents, for confirming every deadline against your denial letter and plan documents (the dates in your denial letter control), and for submitting the packet to your insurer. If your situation involves urgent care, contact your insurer and provider about expedited review right away rather than waiting on us.
Your health information
Documents you send for a packet may contain health information. We use them only to prepare your packet, we never sell them, and access is limited to the people preparing your packet. We keep your source documents while we build your packet and for 30 days after delivery in case follow-up questions come up, then we delete them. This section governs the paid packet service; the free Ask Cura feature remains anonymous as described in the Privacy Policy.
Fees and refunds
The fee is $349 for an internal appeal packet and $599 if the case goes to external review, arranged after we review your intake. If we review your documents and conclude a packet is not viable, there is no charge, and any fee already paid is refunded in full. If your packet contains a factual error we made, we will correct it free or refund your fee.
Limit of liability
To the fullest extent allowed by law, our total liability for this service is limited to the amount you paid for your packet. We are not responsible for your insurer's decisions, for deadlines missed because documents reached us late or incomplete, or for indirect or consequential losses.
Governing law and changes
These terms are governed by the laws of the State of New York. We may update these terms; the version posted on this page when you start your packet is the version that applies to it. Questions: contact Cura Well Plan through the Denial Appeal Packet page.
